Hospital Discharge Care Plan
Are you or a family member in hospital and likely to be discharged soon?
Is there a Hospital Discharge Care Plan in place?
Hospitals are under increasing pressure to discharge patients as quickly as possible. This can sometimes put pressure on patients and families to make decisions without all of the necessary information, care and services in place.
It’s important that elderly family members have a Hospital Discharge Care Plan in place when they are discharged from the hospital otherwise their wellbeing could suffer leading to readmission.
No Hospital Discharge Plan in place – act now.
Nellie offers a Hospital Discharge Care Plan Service – Contact Nellie we can help and support you.
Working with all concerned we will prepare a Hospital Discharge Care Plan and organise any additional Care & Support Services that may be required either short, mid or long term.
NHS Continuing Healthcare
NHS Continuing Healthcare means a package of on-going care that is arranged and funded solely by the NHS where the individual has been found to have a ‘primary health need’.
NHS Continuing Healthcare is complex and can be very daunting for those unfamiliar with the process. Initially, a checklist is completed, usually by a healthcare professional.
If this checklist is ‘positive’ then a more formal assessment meeting is called to decide whether there is a ‘primary health need’ and what funding is granted. This meeting is referred to by professionals as a DST.
We can complete a comprehensive Adult Social Care Assessment to support families and service users at a DST meeting or to support in continuing health care appeals.
